ABSTRACT

En el contexto de la pandemia de COVID-19, a partir de 2020, la Unidad Centinela de Enfermedades Tipo Influenza (UC-ETI) (Santa Fe, Argentina) integró la vigilancia clínica y por laboratorio de SARS-CoV-2 y rinovirus (HRV) a la habitual vigilancia de influenza y otros virus respiratorios (OVR). El objetivo de este estudio transversal y retrospectivo fue describir las características clínico-epidemiológicas de casos de ETI de la ciudad de Santa Fe, con diagnóstico de HRV confirmado durante 2020-2021, en el marco de la UC-ETI. Del total de 600 casos de ETI cuyas muestras fueron analizadas, más del 50,0% fueron mujeres; y la mayor proporción se concentró en el grupo de 15 a 39 años (40,2%). El 33,7% registró al menos una comorbilidad o factor de riesgo, siendo la hipertensión arterial, asma, diabetes, obesidad y EPOC las más frecuentes. Además de fiebre y tos, los signos/síntomas predominantes fueron odinofagia, mialgia y cefalea. El porcentaje de positividad fue de 41,3% en 2020, 27,8% en 2021 y 35,5% en 2020-2021. De las muestras positivas del bienio (213), 59,0% fue SARS-CoV-2, 40,0% HRV y 1,0% OVR. La mayor proporción de diagnósticos positivos de SARS-CoV-2 se concentró en pacientes de 15 años y más, y la de HRV en menores de 15. Los periodos en los que predominó la circulación de HRV no lo hizo la de SARS-CoV-2, y viceversa. El aporte a la carga real de las infecciones virales respiratorias y su impacto en la salud pública destaca la importancia de sostener la vigilancia de HRV


In the context of the COVID-19 pandemic, in 2020, the influenza-like illness Sentinel Unit (ILI-SU) (Santa Fe, Argentina), integrated clinical and laboratory surveillance of SARS-CoV-2 and Rhinovirus (HRV) to the usual surveillance of Influenza and other respiratory viruses (ORV). The objective of this cross-sectional and retrospective study was to describe the clinical-epidemiological characteristics of ILI cases of Santa Fe city, who had a confirmed HRV diagnosis during 2020-2021, by the ILI-SU. Of a total of 600 ILI cases whose samples were analyzed, more than 50.0% were women; and the highest proportion was concentrated in the group between 15 and 39 years of age (40.2%). 33.7% of the cases registered at least one comorbidity or risk factor, among which arterial hypertension, asthma, diabetes, obesity and COPD were the most frequent. In addition to fever and cough, the predominant signs/symptoms were sore throat, myalgia and headache. The percentage of positivity was 41.3% in 2020, 27.8% in 2021, and 35.5% in 2020-2021. Of the biennium positive samples (213), 59.0% were SARS-CoV-2, 40.0% HRV, and 1.0% ORV. The highest proportion of SARS-CoV-2 positive diagnoses were concentrated in patients 15 years of age and older, while the highest proportion of HRV positive diagnoses were concentrated in patients under 15 years of age. The periods in which the circulation of HRV predominated did not predominated SARS-CoV-2, and vice versa. The contribution to the real burden of respiratory viral infections and its impact on public health, highlight the importance of sustaining HRV surveillance

Trata-se de um estudo epidemiológico descritivo retrospectivo, realizado a partir dos casos de síndrome respiratória aguda grave notificados em um hospital de referência em reabilitação do Estado de Goiás no período de janeiro a setembro de 2023. Os dados utilizados foram extraídos do SIVEP - Gripe (Sistema de Informação da Vigilância Epidemiológica da Gripe), que integra a base dos sistemas de informação do Departamento de Informática do Sistema Único de Saúde (DATASUS)


This is a retrospective descriptive epidemiological study, carried out based on cases of severe acute respiratory syndrome reported in a reference hospital for rehabilitation in the State of Goiás from January to September 2023. The data used were extracted from SIVEP - Gripe (Influenza Epidemiological Surveillance Information System), which forms part of the information systems base of the Information Technology Department of the Unified Health System (DATASUS)

ABSTRACT

COVID-19 is reported as an extremely contagious disease with common symptoms of fever, dry cough, sore throat, and tiredness. The published literature on incidence and gender-wise prevalence of COVID-19 is scarce in Pakistan. Therefore, the present study was designed to compare the distribution, incubation period and mortality rate of COVID-19 among the male and female population of district Attock. The data were collected between 01 April 2020 and 07 December 2020 from the population of district Attock, Pakistan. A total of 22,962 individuals were screened and 843 were found positive for RT-qPCR for SARS-CoV-2. The confirmed positive cases were monitored carefully. Among the positive cases, the incidence of COVID-19 was 61.7% among males and 38.2% among females. The average recovery period of males was 18.89±7.75 days and females were 19±8.40 days from SARS-CoV-2. The overall mortality rate was 8.06%. The death rate of male patients was significantly higher (P<0.05) compared to female patients. Also, the mortality rate was higher (P<0.05) in male patients of 40-60 years of age compared to female patients of the same age group. Moreover, the mortality rate significantly increased (P<0.05) with the increase of age irrespective of gender. In conclusion, the incidence and mortality rate of COVID-19 is higher in males compared to the female population. Moreover, irrespective of gender the mortality rate was significantly lower among patients aged <40 years.


Covid-19 é relatada como uma doença extremamente contagiosa com sintomas comuns de febre, tosse seca, dor de garganta e cansaço. A literatura publicada sobre incidência e prevalência de Covid-19 com base no gênero é escassa no Paquistão. Portanto, o presente estudo teve como objetivo comparar a distribuição, o período de incubação e a taxa de mortalidade de Covid-19 entre a população masculina e feminina do distrito de Attock. Os dados foram coletados entre 1 de abril de 2020 e 7 de dezembro de 2020 da população do distrito de Attock, Paquistão. Um total de 22.962 indivíduos foi selecionado, e 843 foram considerados positivos para RT-qPCR para SARS-CoV-2. Os casos positivos confirmados foram monitorados cuidadosamente. Entre os casos positivos, a incidência de Covid-19 foi de 61,7% no sexo masculino e 38,2% no feminino. O período médio de recuperação dos homens foi de 18,89 ± 7,75 dias e das mulheres 19 ± 8,40 dias do SARS-CoV-2. A mortalidade geral foi de 8,06%. A taxa de mortalidade de pacientes do sexo masculino foi significativamente maior (P < 0,05) em comparação com pacientes do sexo feminino. Além disso, a taxa de mortalidade foi maior (P < 0,05) em pacientes do sexo masculino com 40-60 anos de idade em comparação com pacientes do sexo feminino da mesma faixa etária. Além disso, a taxa de mortalidade aumentou significativamente (P < 0,05) com o aumento da idade, independentemente do sexo. Em conclusão, a incidência e a taxa de mortalidade de Covid-19 são maiores no sexo masculino em comparação com a população feminina. E também, independentemente do sexo, a taxa de mortalidade foi significativamente menor entre os pacientes com idade < 40 anos.

Rationale: Since December 2019, the novel coronavirus SARS-Cov-2, also called COVID-19, has spread rapidly across countries, making it one of the biggest health challenges of this century. In Brazil, it was declared a public health emergency in March 2020. The aim of this study was to describe the profile of patients hospitalized by COVID-19 in an emergency hospital in the city of Rio de Janeiro, as well as the factors associated with in-hospital death. Methods: Retrospective observational study, which included patients hospitalized between March and December 2020 with a confirmed diagnosis of COVID-19. The epidemiological, clinical, and laboratory aspects were extracted from the epidemiological investigation files and the hospital chart. Results: 582 suspected cases of COVID-19 were hospitalized and 317 were confirmed, of which 182 (57.5%) were male, and most were residents in the north of Rio de Janeiro (42.5%). Main tomographic or radiological findings: ground glass (34.7%) and pulmonary infiltrate (15.4%), and more than half of those hospitalized (64.0%) had at least one comorbidity. Among hospitalized patients, the overall lethality was 53.6%, and among those admitted to the ICU, this percentage was 84.5%. Age and use of ventilatory support and ICU were the variables that showed a statistically significant association with in-hospital mortality. Conclusion: This study reinforces the importance of epidemiological surveillance, in a hospital setting, especially for diseases in which the passive surveillance system may not be able to adequately report, as in the case of COVID-19.(AU)


Justificativa: Desde dezembro de 2019, o novo coronavírus SARS-Cov-2, também chamado COVID-19, tem se espalhado rapidamente pelos países, tornando-se um dos maiores desafios sanitários deste século. No Brasil, ele foi declarado como una emergência de saúde pública em março de 2020. O objetivo deste estudo foi descrever o perfil dos pacientes hospitalizados por COVID-19 em um hospital de emergência no município de Rio de Janeiro, bem como os fatores associados ao óbito hospitalar. Métodos: Estudo observacional retrospectivo, que incluiu pacientes internados entre março e dezembro de 2020 com um diagnóstico confirmado de COVID-19. Os aspectos epidemiológicos, clínicos, e laboratoriais foram extraídos das fichas de investigação epidemiológica e do prontuário hospitalar. Resultados: Foram internados 582 casos suspeitos de COVID-19 e 317 foram confirmados, dos quais 182 (57,5%) eram do sexo masculino, e a maioria era residente na zona norte do Rio de Janeiro (42,5%). Principais achados 'tomográficos ou radiológicos': vidro fosco (34,7%) e infiltrado pulmonar (15,4%), e mais da metade dos hospitalizados (64,0%) apresentava pelo menos uma comorbidade. Entre os pacientes hospitalizados, a letalidade geral foi de 53,6% sendo que entre os internados na UTI esse percentual foi de 84,5%. Idade e uso de suporte ventilatório e UTI foram as variáveis que mostraram associação estatisticamente significante com mortalidade intra-hospitalar. Conclusão: Este estudo reforça a importância da vigilância epidemiológica, em âmbito hospitalar, principalmente para as doenças em que o sistema de vigilância passivo pode não ser capaz de reportar adequadamente, como no caso da COVID-19.(AU)


Justificación: Desde diciembre de 2019, el coronavirus SARS-Cov-2, también llamado COVID-19, se ha extendido rápidamente por los países, convirtiéndose en uno de los mayores retos sanitarios de este siglo. En Brasil, se declaró una emergencia de salud pública en febrero de 2020. El objetivo de este estudio es describir el perfil de los casos hospitalizados por COVID-19 en un hospital de urgencias de la ciudad de Río de Janeiro, así como los factores asociados a la muerte hospitalaria. Métodos: Estudio observacional, retrospectivo, que incluyó pacientes hospitalizados entre marzo y diciembre de 2020 con diagnóstico confirmado de COVID-19. Los aspectos epidemiológicos, clínicos y de laboratorio fueron extraídos del formulario de investigación epidemiológica y de los registros hospitalarios. Resultados: Se hospitalizaron 582 casos sospechosos de COVID-19 y se confirmaron 317, de los cuales 203 (57,5%) eran hombres, la mayoría residentes en el norte de Río de Janeiro (42,5%). Los principales hallazgos tomográficos o radiológicos: vidrio deslustrado (34,7%) e infiltrado pulmonar (15,4%) y más de la mitad de los hospitalizados (64%) tenían al menos 1 comorbilidad. La letalidad global entre los hospitalizados fue del 53,6% y entre los ingresados en la UCI, este porcentaje fue del 84,5%. Las variables que mostraron una asociación estadísticamente significativa con la mortalidad intrahospitalaria fueron la edad, el uso de soporte ventilatorio y el uso de la UCI. Conclusiones: El estudio refuerza la importancia de la vigilancia epidemiológica en el ámbito hospitalario, especialmente para aquellas enfermedades en las que el sistema de vigilancia pasiva puede no informar adecuadamente, como es el caso de la COVID-19.(AU)

El SARS COV 2, tomó por sorpresa al mundo, con impacto en el sector salud, generándose una gran crisis sanitaria, golpeados por escasez de insumos, de equipos, de personal y capacidad instalada insuficiente para la atención de la contigencia. Ademas, la infodemia, el pánico y el miedo con sus respectivas consecuencias, se empodero de la sociedad civil, situación que no es ajena a sector salud, por ello, este relato de experiencia tuvo objetivo describir la gestión del servicio de medicina crítica de un hospital de Guayaquil- Ecuador durante la pandemia por el Coronavirus. En los centros asitenciales, el personal se enfrenta all desafío sanitario, principalmente en la gestión del servicio de medicina crítica. Al ser nombrado hospital centinela, con una capacidad instadala redujo de 494 a 200 camas, y se creó además el área de hospitalización de infectología; La interrelación de profesionales de la enfermería, médicos infectólogos, médicos neumólogos, servicios de terapia física y rehabilitación, servicios de nutrición, y los terapistas respiratorios, fue fundamental para afrontar la crisis, para vela por el bienestar del paciente, no solo en la parte física, sino psicología y de humanización. No obtante, el miedo a lo desconocido inherente a la especie humana, se transforma en pánico ante esta enfermedad provocando emociones, sentimientos, vivencias exacerbadas, y la inseguridad y desconfianza en que el sistema sanitario(AU)


SARS COV 2 took the world by surprise, with an impact on the health sector, generating a major health crisis, hit by a shortage of supplies, equipment, personnel and insufficient installed capacity for contingency care. In addition, the infodemic, panic and fear with their respective consequences, empowered civil society, a situation that is not unrelated to the health sector, therefore, this experience report aimed to describe the management of the critical medicine service of a Guayaquil-Ecuador hospital during the Coronavirus pandemic. In care centers, the staff faces the health challenge, mainly in the management of the critical medicine service. Being named a sentinel hospital, with an installed capacity it was reduced from 494 to 200 beds, and the infectious disease hospitalization area was also created; The interrelation of nursing professionals, infectious disease doctors, pulmonologists, physical therapy and rehabilitation services, nutrition services, and respiratory therapists, was essential to face the crisis, to ensure the well-being of the patient, not only in the physics, but psychology and humanization. However, the fear of the unknown inherent in the human species is transformed into panic in the face of this disease, causing emotions, feelings, exacerbated experiences, and insecurity and distrust in the health system(Au)

ABSTRACT

Objetivos: descrever o perfil epidemiológico de gestantes e puérperas com Síndrome Respiratória Aguda Grave (SRAG) por COVID-19 no Amazonas. Métodos: trata-se de um estudo quantitativo, transversal que extraiu dados de gestantes e puérperas residentes no estado do Amazonas que foram notificadas no Sistema de Informação da Vi-gilância Epidemiológica da Gripe (SIVEP-Gripe) e tiveram diagnóstico confirmado de SRAG por COVID-19 de 16 de fevereiro de 2020 a 31 de dezembro de 2021. Foram coletados dados sociodemográficos e características clínicas como internação em Unidade de Terapia Intensiva, uso de suporte ventilatório, uso de terapia antiviral e desfechos. Os dados foram analisados e apresentados em frequências absolutas e relativas. Resultados e Discussão: duran-te o período pesquisado, o Amazonas registrou 828 casos de SRAG por COVID-19 em gestantes e puérperas. Des-tes, 58 casos permaneciam em andamento, sem desfecho. A maioria das mulheres tinha idade entre 20 e 34 anos, cor parda, ensino médio, residiam em zona urbana e estavam no terceiro trimestre da gestação no momento do diagnóstico. Em relação às manifestações clínicas, houve predominância de tosse, desconforto respiratório, febre e dispneia. As comorbidades mais frequentes foram asma, doença cardiovascular crônica, diabetes mellitus e obe-sidade. Em relação aos dados clínicos, 19,7% das gestantes e puérperas foram internadas em UTI. Destas, 70,6% utilizaram ventilação mecânica invasiva, 80,2% fizeram uso de terapia antiviral e 79 (10,3%) evoluíram para óbito. Conclusão: o estudo permitiu a caracterização do perfil epidemiológico de gestantes e puérperas com SRAG por COVID-19 no Amazonas e demonstrou taxa elevada de óbito no período estudado, sobretudo na capital. Esse perfil pode ter relação com os determinantes sociais da saúde e as dificuldades de acesso aos serviços de saúde podem ter papel fundamental na assistência. Com isso, o atendimento às gestantes e puérperas com SRAG por COVID-19 deve ser prioridade nos serviços de saúde, principalmente nos casos em que as pacientes possuem comorbidades (AU)


Objectives: To describe the epidemiological profile of pregnant and postpartum women with severe acute res-piratory syndrome (SARS) caused by severe acute respiratory syndrome corona virus (SARS-CoV-2) in Amazonas. Methods: This quantitative, cross-sectional study extracted data from pregnant and postpartum women residing in the state of Amazonas who were notified in the Influenza Epidemiological Surveillance Information System (SIVEP-Gripe) and had a confirmed diagnosis of SARS during corona virus disease (COVID-19) from February 16, 2020, to December 31, 2021. Sociodemographic data and clinical characteristics, such as admission to the intensive care unit, use of ventilatory support, use of antiviral therapy, and outcomes were collected. Data were analyzed and presented with relative frequencies. Results and Discussion: During the study period, Amazonas recorded 828 cases of SARS caused by SARS-CoV-2 in pregnant and postpartum women. Of these, 58 patients remained in progress with no outcomes. Most women were aged between 20 and 34 years, educated to high school level, lived in urban areas, and were in the third trimester of pregnancy at the time of diagnosis. Predominant clinical manifes-tation was cough, respiratory distress, fever, and dyspnea. The most frequent comorbidities were asthma, chronic cardiovascular diseases, diabetes mellitus, and obesity. 19.7% of pregnant and postpartum women were admitted to the intensive care unit (ICU). Of these, 70.6% needed invasive mechanical ventilation, 80.2% antiviral therapy, and 79 patients (10.3%) died. Conclusion: This study characterized the epidemiological profile of pregnant and postpartum women with SARS caused by SARS-CoV-2 in Amazonas and showed a high death rate during the study period, especially in the capital (AU)

Objetivo: Analisar a completude das notificações de casos de síndrome respiratória aguda grave no Sistema de Informação de Vigilância Epidemiológica da Gripe (SIVEP-Gripe) durante a pandemia de COVID-19, na base de dados nacional e na base da Unidade Regional de Saúde do estado de Minas Gerais, Brasil, em 2020. Métodos: Estudo descritivo da completude das variáveis sociodemográficas e das relativas à etiologia, condição clínica, evolução e critérios diagnósticos do SIVEP-Gripe. O nível de completude foi classificado como excelente (> 95%), bom (90% a 95%), regular (80% a 90%), ruim (50% a 80%) ou muito ruim (< 50%). Resultados: O percentual de variáveis com completude excelente foi de apenas 18,1% na base de dados nacional, e de 27,8% na base de dados regional. Conclusão: Evidenciou-se baixa completude de ambas as bases de dados do SIVEP-Gripe, tornando-se necessários aperfeiçoamentos no processo de trabalho e capacitações rotineiras dos profissionais para o correto preenchimento.


Resumen Objetivo: Analizar la completitud de las notificaciones de casos de síndrome respiratorio agudo severo del Sistema de Información de Vigilancia Epidemiológica de la Gripe (SIVEP-Gripe) durante la pandemia de COVID-19, en la base de datos nacional y en una base de datos regional de salud en el estado de Minas Gerais, Brasil, en 2020. Métodos: Estudio descriptivo de la completitud de las variables sociodemográficas y las relacionadas con la etiología, cuadro clínico, evolución y criterios diagnósticos del SIVEP-Gripe. El nivel de completitud se clasificó como excelente (> 95%), bueno (90% a 95%), regular (80% a 90%), malo a (50% a 80%) y muy malo (< 50%). Resultados: El porcentaje de variables con excelente completitud fue solo del 18,1% en la base de datos nacional y del 27,8% en la base de datos regional. Conclusión: Se evidenció la baja completitud de ambas bases de datos SIVEP-Gripe, lo que hace necesario mejorar el proceso de trabajo y la rutina de capacitación de los profesionales.

Conclusion: Both SIVEP-Gripe databases presented low completeness, making it necessary to improve the work process and routine training of professionals aimed at the correct filling.

ABSTRACT BACKGROUND: The COVID-19 pandemic has instilled fear and stress among healthcare workers. OBJECTIVES: The aim of this study was to assess work stress and associated factors among healthcare workers during the COVID-19 outbreak and to evaluate whether prior experience of treating severe acute respiratory syndrome (SARS) had a positive or negative influence on healthcare workers' stress levels during the COVID-19 pandemic. DESIGN AND SETTING: Cross-sectional survey in a tertiary hospital in Kaohsiung City, in southern Taiwan. METHODS: The survey was conducted using an online self-administered questionnaire to measure the stress levels among healthcare workers from March 20 to April 20, 2020. The stress scales were divided into four subscales: worry of social isolation; discomfort caused by the protective equipment; difficulties and anxiety regarding infection control; and workload of caring for patients. RESULTS: The total stress scores were significantly higher among healthcare workers who were aged 41 or above, female, married, parents and nurses. Those with experience of treating SARS reported having significantly higher stress scores on the subscale measuring the discomfort caused by protective equipment and the workload of caring for patients. During the COVID-19 outbreak, frontline healthcare workers with experience of treating SARS indicated having higher stress levels regarding the workload of caring for patients than did non-frontline healthcare workers with no experience of treating SARS. CONCLUSIONS: Work experience from dealing with the 2003 SARS virus may have had a negative psychological impact on healthcare workers amidst the COVID-19 outbreak.

Objetivo: Descrever o perfil clínico-epidemiológico dos casos suspeitos de COVID-19 internados em hospital federal do Rio de Janeiro, RJ, Brasil, e identificar fatores associados ao óbito. Métodos: Estudo seccional, utilizando dados da vigilância epidemiológica local até a Semana Epidemiológica 27 de 2020. Empregou-se regressão logística. Resultados: Foram incluídos 376 casos internados suspeitos de COVID-19, dos quais 52,9% eram mulheres, 57,4% tinham 50 anos ou mais de idade e 80,1% exibiam comorbidades. Foram confirmados para COVID-19 195 casos (51,9%), cuja letalidade (37,9%) foi maior, comparada à dos descartados (24,2%). Na análise ajustada, associaram-se ao óbito, nos casos confirmados, ter 50 a 69 (OR=11,65 - IC95% 1,69;80,33) e 70 ou mais anos (OR=8,43 - IC95% 1,22;58,14), apresentar neoplasia (OR=4,34 - IC95% 1,28;14,76) e usar suporte ventilatório invasivo (OR=70,20 - IC95% 19,09;258,19). Conclusão: Houve elevada prevalência de comorbidades e letalidade; os principais fatores associados ao óbito foram idade, presença de neoplasia e suporte ventilatório invasivo.


Objetivo: Describir el perfil clínico-epidemiológico de casos sospechosos de COVID-19 ingresados en hospital federal de Río de Janeiro, RJ, Brasil, y factores asociados al óbito. Métodos: Estudio transversal utilizando datos de la vigilancia epidemiológica local de casos sospechosos hospitalizados hasta la Semana Epidemiológica 27 de 2020 y regresión logística. Resultados: Se incluyeron 376 casos, de los cuales 52,9% era de mujeres, 57,4% tenía 50 años o más, 80,1% tenía comorbilidades. Se confirmó para COVID-19 un 51,9% . Los casos confirmados tuvieron una mayor letalidad (37,9%) que los descartados (24,2%). La mortalidad ajustada en los confirmados fue mayor en los grupos de edad de 50 a 69 años (OR=11,65 - IC95% 1,69; 80,33), 70 años o más (OR=8,43 - IC95% 1,22;58,14), con neoplasia (OR=4,34 - IC95% 1,28;14,76) y uso de soporte ventilatorio invasivo (OR=70,20 - IC95% 19,09;258,19). Conclusión: La mayor mortalidad se asoció con personas de edad avanzada, con neoplasias y uso de soporte ventilatorio invasivo.


Objective: To describe the clinical and epidemiological profile of suspected COVID-19 cases admitted to a federal hospital in Rio de Janeiro, RJ, Brazil, and to identify factors associated with death. Methods:This was a cross-sectional study using local epidemiological surveillance data as at epidemiological week 27 of 2020 and logistic regression. Results: 376 hospitalized suspected COVID-19 cases were included; 52.9% were female, 57.4% were 50 years old or over and 80.1% had comorbidities. 195 (51.9%) COVID-19 cases were confirmed and their lethality was higher (37.9%) than among discarded cases (24.2%). In the adjusted analysis, death among confirmed cases was associated with being in the 50-69 age group (OR=11.65 - 95%CI 1.69;80.33), being aged 70 or over (OR=8.43 - 95%CI 1.22;58.14), presence of neoplasms (OR=4.34 - 95%CI 1.28;14.76) and use of invasive ventilatory support (OR=70.20 - 95%CI 19.09;258.19). Conclusion: High prevalence of comorbidities and lethality was found; the main factors associated with death were being older, neoplasms and invasive ventilatory support.

INTRODUCTION@#We evaluated the impact of public health measures on paediatric emergency department attendances during the COVID-19 and severe acute respiratory syndrome (SARS) outbreaks in Singapore.@*METHODS@#Between 1 January 2020 and 31 July 2020, we retrospectively reviewed paediatric emergency department attendances and admissions in a tertiary paediatric hospital in Singapore before and after a national lockdown to combat the spread of COVID-19 in Singapore. Hospital attendances and admissions were compared with data from a corresponding period in 2019 (1 January 2019 to 31 July 2019), as well as during and after the SARS outbreak (1 January 2003 to 31 December 2004).@*RESULTS@#Compared with a corresponding non-outbreak period, emergency department attendances decreased in line with nationwide public health measures during the COVID-19 and SARS outbreaks (2020 and 2003 respectively), before increasing gradually following lifting of restrictions, albeit not to recorded levels before these outbreaks. During the COVID-19 outbreak, mean daily attendances decreased by 40%, from 458 per day in January-July 2019, to 274 per day in January-July 2020. The absolute number of hospital inpatient admissions decreased by 37% from January-July 2019 (19,629) to January-July 2020 (12,304). The proportion of emergency department attendances requiring admission remained similar: 20% in January-July 2019 and 21% in January-July 2020.@*CONCLUSION@#Nationwide public health measures in Singapore have had an impact on paediatric emergency department attendances and hospital inpatient admissions. Data from this study could inform planning and resource allocation for emergency departments in Singapore and internationally.

ABSTRACT

Resumen La emergencia del nuevo SARS-CoV-2 - y el reconocimiento de la enfermedad que produce como pandemia - constituye el evento sanitario de mayor importancia en 2020. Debido a su reciente aparición y la inexistencia de medicamentos o vacunas seguras y eficaces, el manejo de casos y el control de brotes de la enfermedad se hace difícil. En esta revisión, se expone información sobre la pandemia, las características del virus, las manifestaciones clínicas de la enfermedad, su adecuado diagnóstico, las alternativas de tratamiento y las acciones que se ejecutan para su control (AU).


Abstract The emergency of the novel coronavirus, and the recognition of the disease it causes as a pandemic, constitute the main sanitary event in 2020. Because its recent discovery and the lack of safe and effective drugs or vaccines, the managing of cases and the control of the pandemic is problematic. In this review, the authors summarize the basic characteristics of the pathogen, the signs and symptoms of disease, and the diagnosis tools to identify presence of the virus. Furthermore, we also review the current knowledge on the treatment of infected cases as well as information about vaccine candidates. We highlight actions executed in Cuba to control the pandemic (AU).

ABSTRACT

ABSTRACT The emergence of coronavirus disease 19 pandemic and novel research on the high transmissibility of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has raised controversies over the use of face masks to prevent community transmission. Specific regulations need to be fulfilled to use a face mask as part of the personal protective equipment and high quality of evidence supporting its use to prevent respiratory viral infections, including SARS-CoV-2, is lacking. However, its widespread use is becoming a standard practice in some countries and discrepancies between health authorities on their policy have led to controversy. The aim of this review is to provide an outlook on recent research in this matter and areas of opportunity.

Resumo O objetivo deste artigo é avaliar a eficácia das máscaras faciais padrão tecido não tecido (TNT) para a prevenção de doenças respiratórias (MERS CoV, SARS-CoV e SARS-CoV-2) na população. Foi realizada busca nas bases de dados Medline, Embase, Cinahl, The Cochrane Library, Trip. Também busca complementar no Google Acadêmico, Rayyan e medRxiv. Não foram aplicados filtros relacionados a data, idioma ou status de publicação. Títulos e resumos foram rastreados e, posteriormente, textos completos foram avaliados. Foram incluídos três estudos: um ensaio clínico randomizado tipo cluster e duas revisões sistemáticas. O ensaio clínico indica benefício potencial de máscaras médicas para controle da fonte de infecção, para a doença respiratória clínica. Em uma das revisões sistemáticas, não foi possível estabelecer relação conclusiva entre uso da máscara e proteção contra infecção respiratória. Por fim, outra revisão sistemática demonstrou que máscaras são eficazes na prevenção da propagação de vírus respiratórios. As evidências apontam para benefício potencial das máscaras faciais padrão TNT. Para o cenário atual de pandemia por COVID 19, recomenda-se educação sobre uso adequado de máscaras, associado a medidas individuais de proteção.


Abstract Objectives: to evaluate the effectiveness of non-woven face masks for the prevention of respiratory infections (MERS CoV, SARS-CoV, and SARS-CoV-2) in the population. Methods: search in Medline, Embase, Cinahl, The Cochrane Library, Trip databases. Google Scholar, Rayyan and medRxiv were also consulted for complementary results. No filters related to date, language or publication status were applied. Titles and abstracts were screened, and later, full texts were evaluated. Results: three studies were included: a randomized cluster clinical trial and two systematic reviews. The clinical trial indicates a potential benefit of medical masks to control the source of clinical respiratory disease infection. In one of the systematic reviews, it was not possible to establish a conclusive relationship between the use of the mask and protection against respiratory infection. Finally, another systematic review indicated that masks are effective in preventing the spread of respiratory viruses. Conclusion: Evidence points to the potential benefit of standard non-woven face masks. For the current pandemic scenario of COVID-19, education on the appropriate use of masks associated with individual protection measures is recommended.

Resumo Foi analisado o padrão da Síndrome Respiratória Aguda Grave (SRAG) em Pernambuco antes e durante período pandêmico da COVID-19. Estudo ecológico, de janeiro a junho de 2015 a 2019 (período pré-pandemia) e 01 de janeiro a 15 de junho de 2020, período que contempla parte inicial da pandemia. Taxas de detecção por município e Regional de Saúde de residência foram calculadas e, por meio da razão de risco, a dinâmica espacial da SRAG foi estimada. Na pré-pandemia ocorreram 5.617 casos de SRAG, 187 casos/mês e 23,8 casos/100 mil hab. e, na pandemia, 15.100 casos, 2.516 casos/mês e 320,3 casos/100 mil hab., um aumento da detecção mensal em 13 vezes. Foi ampliada (p < 0,001) a ocorrência em idosos (de 7,7% para 48,3%), a coleta de amostras (de 60,9% para 96,5%) e a identificação do agente etiológico causador da SRAG, com predomínio da SRAG por COVID-19 (66,0%). Regiões do interior do estado tiveram razão de detecção de SRAG 20 vezes superior à esperada, sendo o excesso de risco associado a menor IDHM, a condição do município ser sede de Regional de Saúde e a presença de rodovia federal no território municipal. Conhecer a mudança do padrão da ocorrência da SRAG, aliada a análise espacial poderão contribuir para o planejamento de ações a curto e médio prazo nos diferentes níveis de gestão.


Abstract The aim of this study was to analyze the Severe Acute Respiratory Syndrome (SARS) pattern in Pernambuco before and during a COVID-19 pandemic. Ecological study conducted from January to June, 2015 to 2019 and from January 1 to June 15, 2020. The detection rates by municipality and by Regional Health of residence were calculated. The spatial area of SARS was estimated through the risk ratio. Before the pandemic, there were 5,617 cases of SARS, 187 cases/month and 23.8 cases/100 thousand inhabitants, while during the pandemic there were 15,100 cases, 2,516 cases/month and 320.3 cases/100 thousand inhabitants, which represents a 13-fold increase in detection. The following expanded (p < 0,001): the occurrence in elderly people, the collection of samples and the identification of SARS etiological agent with predominance of SARS by COVID-19. Most municipalities experienced a 20-fold higher detection than expected, suggesting a process of virus spread to the hinterlands. The excess risk associate with lower IDHM, the condition of the municipality being the headquarters of the Regional Health and the presence of a highway in the municipality. The change in the pattern of occurrence of SRAG, combined with Spatial analysis may contribute to action planning at different levels of management.

Métodos: Estudo transversal, realizado com dados das fichas de notificação de SRAG de crianças e adolescentes (0 a 19 anos) com confirmação laboratorial para COVID-19. Foram incluídas as notificações com evolução completa de SRAG por COVID-19 até a 38ᵃ Semana Epidemiológica de 2020. Resultados: Foram investigadas 6.989 hospitalizações, das quais 661 evoluíram a óbito, perfazendo uma letalidade hospitalar de 9,5%. Observou-se maior letalidade entre menores de 1 ano de idade (14,2%), crianças e adolescentes do sexo feminino (9,7%), indígenas (23,0%) e residentes em zonas rurais (18,1%), como também nas regiões Nordeste (15,4%) e Norte (9,7%) do país. Conclusão: Foram observadas diferenças na letalidade hospitalar, conforme as características sociodemográficas e marcantes desigualdades regionais.

Métodos: Estudio transversal realizado con datos de los formularios de notificación de SRAS de niños y adolescentes (0 a 19 años) confirmados en laboratorio para COVID-19. Se incluyeron notificaciones con evolución completa del SRAS por COVID-19, hasta la 38ª Semana Epidemiológica de 2020. Resultados: Se incluyeron 6.989 hospitalizaciones, 661 fallecidas, resultando en letalidad hospitalaria del 9,5%. Se observaron mayores tasas de letalidad entre los niños menores de un año (14,2%), niñas y adolescentes (9,7%), indígenas (23,0%) y residentes en zonas rurales (18,1%), así como en las regiones Nordeste (15,4%) y Norte (9,7%). Conclusión: Se observaron diferencias en la mortalidad hospitalaria según las características sociodemográficas y marcadas desigualdades regionales.


Objective: To describe the profile of deaths and the lethality of Severe Acute Respiratory Syndrome (SARS) due to COVID-19 in hospitalized children and adolescents in Brazil. Methods: This was a cross-sectional study conducted with data from the SARS notification forms of children and adolescents (0 to 19 years old) with laboratory-confirmed COVID-19. Notifications with complete progression of SARS due to COVID-19 were included, up to the 38th Epidemiological Week of 2020. Results: 6,989 hospitalizations were investigated, 661 died, resulting in 9.5% hospital lethality. Higher lethality rates were observed among children under 1 year of age (14.2%), female children and adolescents (9.7%), the indigenous (23.0%), and those living in rural areas (18.1 %), as well as in the Northeast (15.4%) and North (9.7%) regions of Brazil. Conclusion: Differences in hospital mortality were found according to sociodemographic characteristics and marked regional inequalities.

Objetivo: avaliar os atributos do sistema de vigilância da síndrome respiratória aguda grave (SRAG) no Brasil, no período 2014-2016. Métodos: estudo de avaliação segundo o guia do Centro de Controle de Doenças (Centers for Disease Control and Prevention - CDC) dos Estados Unidos; utilizaram-se dados do sistema de informação Influenza Web, notificados entre 2014 e 2016; avaliaram-se os atributos simplicidade, completude, inconsistência, oportunidade, aceitabilidade, representatividade, valor preditivo positivo (VPP) da definição de caso de SRAG e utilidade. Resultados: observou-se estrutura simples; boa completude (100% nas variáveis obrigatórias; >95% nas não obrigatórias); baixa inconsistência (3,2%); não oportunidade (68,2%); aceitabilidade baixa (média de 70,4%); representatividade do território (capaz de análises de grupos de risco); alto VPP (29,1%); e utilidade (por cumprir objetivos do sistema). Conclusão: os atributos avaliados indicam que o sistema é capaz de fornecer informações completas, representativas e úteis sobre influenza, adequadas para balizar respostas do sistema de saúde.

Método: estudio de evaluación según la guía del Centro para el Control y la Prevención de Enfermedades (Centers for Disease Control and Prevention - CDC) dos Estados Unidos. Se utilizaron datos del sistema de información Influenza Web, notificados entre 2014-2016. Se evaluaron los atributos simplicidad, completitud, inconsistencia, oportunidad, aceptabilidad, representatividad, valor predictivo positivo (VPP) de definición de caso de SARS y utilidad. Resultados: se observó estructura simple; buena completitud (100% en variables obligatorias; >95% en variables no obligatorias); baja inconsistencia (3.2%); no oportunidad (68,2%); baja aceptabilidad (promedio de 70.4%); representatividad de territorio (capaz de analizar grupos de riesgo); VPP alto (29.1%); y utilidad (por cumplir los objetivos del sistema). Conclusión: los atributos evaluados indican que el sistema es capaz de suministrar informaciones completas, representativas y útiles sobre la influenza en Brasil, adecuadas para orientar respuestas del sistema de salud.


Objective: to evaluate attributes of the severe acute respiratory syndrome (SARS) surveillance system in Brazil, 2014-2016. Method: this was an evaluation study conducted according to United States Centers for Disease Control and Prevention guidelines. Data from the Influenza Web information system notified for the period 2014-2016 were used. The simplicity, completeness, inconsistency, timeliness, acceptability, representativeness, positive predictive value (PPV) of the SARS case definition attributes and usefulness were evaluated. Results: a simple structure was found with good completeness (100% for required variables; >95% for optional variables); low inconsistency (3.2%); lack of timeliness (68.2%); low acceptability (average of 70.4%); representative of the territory (capable of analyzing risk groups); high PPV (29.1%); useful (fulfils system objectives). Conclusion: the attributes evaluated indicate that the system is capable of providing complete, representative and useful information about influenza, adequate for guiding national health responses.

ABSTRACT

O presente estudo tem o objetivo de descrever os pacientes hospitalizados por síndrome respiratória aguda grave (SRAG) em decorrência da COVID-19 (SRAG-COVID), no Brasil, quanto às suas características demográficas e comorbidades até a 21ª Semana Epidemiológica de 2020. Buscou-se comparar essas características com as dos hospitalizados por SRAG em decorrência da influenza em 2019/2020 (SRAG-FLU) e com a população geral brasileira. As frequências relativas das características demográficas, comorbidades e de gestantes/puérperas entre os pacientes hospitalizados por SRAG-COVID e SRAG-FLU foram obtidas por meio do Sistema de Informação de Vigilância Epidemiológica da Gripe (SIVEP-Gripe), e as estimativas para a população geral brasileira foram obtidas por meio de projeções populacionais realizadas pelo Instituto Brasileiro de Geografia e Estatística, dados do Sistema de Informações sobre Nascidos Vivos e de pesquisas de âmbito nacional. Entre os hospitalizados por SRAG-COVID, observou-se uma elevada proporção, em relação ao perfil da população geral brasileira, de indivíduos do sexo masculino, idosos ou com 40 a 59 anos, com comorbidades (diabetes mellitus, doença cardiovascular, doença renal crônica e pneumopatias crônicas) e de gestantes/puérperas. Já entre os hospitalizados por SRAG-FLU, observou-se prevalências superiores às populacionais de indivíduos de 0 a 4 anos de idade ou idosos, de raça ou cor branca, com comorbidades (diabetes mellitus, doença renal crônica, asma e outras pneumopatias crônicas) e de gestantes/puérperas. Esses grupos podem estar evoluindo para casos mais graves da doença, de forma que estudos longitudinais na área são de extrema relevância para investigar esta hipótese e melhor subsidiar políticas públicas de saúde.


El objetivo del presente estudio es describir a los pacientes hospitalizados por infección respiratoria aguda grave (IRAG) a consecuencia de la COVID-19 (IRAG-COVID), en Brasil, respecto a sus características demográficas y comorbilidades hasta la 21ª Semana Epidemiológica de 2020. Se buscó comparar estas características con las de los hospitalizados por SRAS, a consecuencia de la influenza en 2019/2020 (IRAG-FLU) y con la población general brasileña. Las frecuencias relativas de las características demográficas, comorbilidades y de embarazadas/puérperas entre los pacientes hospitalizados por IRAG-COVID y IRAG-FLU se obtuvieron mediante el Sistema de Información de la Vigilancia Epidemiológica de la Gripe (SIVEP-Gripe), y las estimaciones para la población general brasileña se consiguieron mediante proyecciones poblacionales realizadas por el Instituto Brasileño de Geografía e Estadística, datos del Sistema de Informaciones sobre Nascidos Vivos y de investigaciones de ámbito nacional. Entre los hospitalizados por IRAG-COVID, se observó una elevada proporción, respecto al perfil de la población general brasileña, de individuos del sexo masculino, ancianos o con 40 a 59 años, con comorbilidades (diabetes mellitus, enfermedad cardiovascular, enfermedad renal crónica y neumopatías crónicas) y de embarazadas/puérperas. Ya entre los hospitalizados por IRAG-FLU, se observaron prevalencias superiores a las poblacionales de individuos de 0 a 4 años de edad o ancianos, de raza o color blanco, con comorbilidades (diabetes mellitus, enfermedad renal crónica, asma y otras neumopatías crónicas) y de embarazadas/puérperas. Estos grupos pueden estar evolucionando hacia casos más graves de la enfermedad, por ello, los estudios longitudinales en esta área son de extrema relevancia para investigar esta hipótesis y apoyar mejor las políticas públicas de salud.


The study aims to describe patients hospitalized for severe acute respiratory illness (SARI) due to COVID-19 (SARI-COVID) in Brazil according to demographic characteristics and comorbidities up to the 21st Epidemiological Week of 2020. The study aimed to compare these characteristics with those of patients hospitalized for SARI due to influenza in 2019/2020 (SARI-FLU) and with the Brazilian general population. The proportions of demographic characteristics, comorbidities, and pregnant and postpartum women among patients hospitalized for SARI-COVID and SARI-FLU were obtained from the SIVEP-Gripe database, and the estimates for the Brazilian population were obtained from the population projections performed by Brazilian Institute of Geography and Statistics, Information System on Live Birth data, and nationwide surveys. Compared to the Brazilian population, patients hospitalized for SARI-COVID showed a higher proportion of males, elderly individuals and those aged 40 to 59 years, comorbidities (diabetes mellitus, cardiovascular disease, chronic kidney disease, and chronic lung diseases), and pregnant/postpartum women. Compared to the general population, Brazilians hospitalized for SARI-FLU showed higher prevalence rates of ages 0 to 4 years or over 60 years, white race/color, comorbidities (diabetes, chronic kidney disease, asthma, and other chronic lung diseases), and pregnant/postpartum women. The data suggest that these groups are evolving to more serious forms of the disease, so that longitudinal studies are extremely relevant for investigating this hypothesis and supporting appropriate public health policies.

Resumo: A vigilância de síndrome respiratória aguda grave (SRAG) no Brasil visa a caracterizar a circulação dos vírus Influenza A e B em casos hospitalizados e óbitos, tendo sido ampliada em 2012 para incluir outros vírus respiratórios. A COVID-19 foi detectada no Brasil pela primeira vez na 9ª semana epidemiológica de 2020 e o teste para o vírus SARS-CoV-2 foi incluído no protocolo de vigilância a partir da 12ª semana epidemiológica. O objetivo deste estudo foi investigar o padrão de hospitalizações por SRAG no país após a entrada do SARS-CoV-2, comparando o perfil temporal, etário e de resultados laboratoriais com os anos de 2010 a 2019. Em 2020, a hospitalização por SRAG, contabilizada desde a data do primeiro caso de COVID-19 confirmado até a 12ª semana, superou o observado, no mesmo período, em cada um dos 10 anos anteriores. A faixa etária acima de 60 anos foi a mais acometida, em nível acima do histórico. Houve um aumento considerável de testes laboratoriais negativos, sugerindo a circulação de um vírus diferente dos presentes no painel. Concluímos que o aumento das hospitalizações por SRAG, a falta de informação específica sobre o agente etiológico e a predominância de casos entre idosos, no mesmo período de tempo em que cresce o número de casos novos de COVID-19, é coerente com a hipótese de que os casos graves da doença já estejam sendo detectados pela vigilância de SRAG com sobrecarga para o sistema de saúde. A inclusão da testagem para SARS-CoV-2 no protocolo de vigilância de SRAG e sua efetiva implementação são de grande importância para acompanhar a evolução dos casos graves da doença no país.


Resumen: La vigilancia del síndrome respiratorio agudo grave (SRAG) en Brasil tiene como objetivo caracterizar la circulación de los virus de la Influenza A y B en casos y muertes hospitalizadas, y se expandió en 2012 para incluir otros virus respiratorios. La COVID-19 se detectó en Brasil por la primera vez en la 9ª semana epidemiológica de 2020, y el examen test para el virus SARS-CoV-2 se incluyó en el protocolo de vigilancia a partir de la 12ª semana epidemiológica. El objetivo de este estudio fue investigar el patrón de hospitalizaciones por SRAG en Brasil desde la entrada de SARS-CoV-2, comparando el perfil temporal y de edad y los resultados de laboratorio entre los años 2010 a 2019. En 2020, las hospitalizaciones por SRAG, compiladas a partir de la fecha del primer caso confirmado de COVID-19 hasta la 12ª semana, excedió los números observados durante el mismo período en cada uno de los 10 años anteriores. El grupo de edad mayor de 60 años fue el más afectado, a niveles superiores a los históricos. Hubo un aumento considerable en las pruebas de laboratorio negativas, lo que sugiere la circulación de un virus diferente de los que ya están presentes en el panel. Se concluye que el aumento de las hospitalizaciones por SRAG, la falta de información específica sobre el agente etiológico y el predominio de casos entre los ancianos en el mismo período en que hubo un aumento de casos nuevos de COVID-19 se entiende que con esta hipótesis de que los casos graves de COVID-19 ya estén siendo monitorados por la vigilancia de SRAG, lo que genera una sobrecarga en el sistema de salud. La inclusión de los exámenes para SARS-CoV-2 en el protocolo de vigilancia de SRAG y la eficacia de implementación son de grande importancia para monitorear la evolución de los casos graves de COVID-19 en Brasil.


Abstract: Surveillance of the severe acute respiratory illness (SARI) in Brazil aims to characterize the circulation of the Influenza A and B viruses in hospitalized cases and deaths, having been expanded in 2012 to include other respiratory viruses. COVID-19 was detected in Brazil for the time in the 9th epidemiological week of 2020, and the test for the SARS-CoV-2 virus was included in the surveillance protocol starting in the 12th epidemiological week. This study's objective was to investigate the pattern of hospitalizations for SARI in Brazil since the entry of SARS-CoV-2, comparing the temporal and age profiles and laboratory results to the years 2010 through 2019. In 2020, hospitalizations for SARI, compiled from the date of the first confirmed case of COVID-19 up to the 12th week, exceeded the numbers observed during the same period in each of the previous 10 years. The age bracket over 60 years was the most heavily affected, at higher than historical levels. There was a considerable increase in negative laboratory tests, suggesting circulation of a different virus from those already present in the panel. We concluded that the increase in hospitalizations for SARI, the lack of specific information on the etiological agent, and the predominance of cases among the elderly during the same period in which there was an increase in the number of new cases of COVID-19 are all consistent with the hypothesis that severe cases of COVID-19 are already being detected by SARI surveillance, placing an overload on the health system. The inclusion of testing for SARS-CoV-2 in the SARI surveillance protocol and the test's effective nationwide deployment are extremely important for monitoring the evolution of severe COVID-19 cases in Brazil.

ABSTRACT Background: Influenza continues to drive seasonal morbidity, particularly in settings with low vaccine coverage. Objectives: To describe the influenza cases and viral circulation among hospitalized patients. Methods: A prospective study based on active surveillance of inpatients with influenza-like illness from a tertiary hospital in Bucharest, Romania, in the season 2016/17. Results: A total of 446 patients were tested, with a balanced gender distribution. Overall, 192 (43%) patients tested positive for influenza, with the highest positivity rate in the age groups 3-13 years and >65 years. Peak activity occurred between weeks 1 and 16/2017, with biphasic distribution: A viruses were replaced by B viruses from week 9/2017; B viruses predominated (66.1%). Among the 133 (69.3%) subtyped samples, all influenza A were subtype H3 (n = 57) and all influenza B were B/Victoria (n = 76). Patients who tested positive for influenza presented fewer comorbidities (p = 0.012), except for the elderly, in whom influenza was more common in patients with comorbidities (p = 0.050). Disease evolution was generally favorable under antiviral treatment. The length of hospital stay was slightly longer in patients with influenza-like illness who tested patients negative for influenza (p = 0.031). Conclusions: Distinctive co-circulation of A/H3 and B/Victoria in Bucharest, Romania in the 2016/17 influenza season was found. While the A/H3 subtype was predominant throughout Europe that season, B/Victoria appears to have circulated specifically in Romania and the Eastern European region, predominantly affecting preschoolers and school children.
